Job Description

• Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
• Patient age range: birth to over 100 years.
• Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services.
• Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.
• Weekend rotations occur once every 6 weeks.
• Ability to travel to assigned cases is required.

Licensure & Certifications

• Must have a Physical Therapist professional license valid in Massachusetts (MA Conditional Licenses not accepted).

Prior Travel Experience

• No explicit requirement for prior travel experience indicated.

Required Credentials & Compliance (Selected Highlights)

• Professional license verification and primary source verification required.
• Background checks including criminal background, Massachusetts CORI, SSN trace, national sex offender check, and exclusion checks.
• Motor Vehicle Record (MVR) check required for all home care positions.
• Submission of a recent professional photo (no selfies, plain background).
• Fit Test for N95 respirator within 365 days of start date, updated annually.
• Health requirements including:

  • Influenza vaccine during flu season (Sept 15 - April 30), no exemptions.
  • TB test or blood test within 3 months of start date, updated if >6 month break.
  • MMR vaccination or proof of immunity.
  • Health statement signed by a licensed provider within 365 days.
• Submission of 2 recent references (minimum 1 supervisor or above).
• Skills checklist and candidate submission coversheet required.
• Education verification through background check company.
