Job Description
- Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
- Patient age range: birth to over 100 years.
- Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services.
- Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.
- Weekend rotations occur once every 6 weeks.
- Ability to travel to assigned cases is required.
Licensure & Certifications
- Must have a Physical Therapist professional license valid in Massachusetts (MA Conditional Licenses not accepted).
Prior Travel Experience
- No explicit requirement for prior travel experience indicated.
Required Credentials & Compliance (Selected Highlights)
- Professional license verification and primary source verification required.
- Background checks including criminal background, Massachusetts CORI, SSN trace, national sex offender check, and exclusion checks.
- Motor Vehicle Record (MVR) check required for all home care positions.

- Fit Test for N95 respirator within 365 days of start date, updated annually.
- Health requirements including:
- Influenza vaccine during flu season (Sept 15 - April 30), no exemptions.
- TB test or blood test within 3 months of start date, updated if >6 month break.
- MMR vaccination or proof of immunity.
- Health statement signed by a licensed provider within 365 days.
- Submission of 2 recent references (minimum 1 supervisor or above).
- Skills checklist and candidate submission coversheet required.
- Education verification through background check company.
