Job Opportunity: Registered Nurse (RN) - Home Health (Days)

Job Summary

Provide full-time professional nursing care to patients in their homes, following agency standards and policies. Work under the supervision of the Clinical Services Supervisor.

Key Responsibilities

• Initiate and provide medically prescribed skilled nursing care and health counseling to support treatment, rehabilitation, and symptom management.
• Conduct initial patient visits to assess physical and emotional status, needs, and resources through observation, interviews, and record analysis.
• Prepare nursing care plans aligned with physician treatment plans and provide patient education based on the Care Plan/Standard of Care.

Requirements

• Specialty: Registered Nurse (RN)
• Experience: Minimum 2+ years of nursing experience (highest required)
• Skills:

  • Infusion and wound care (required)
  • Experience with OASIS documentation (required)
  • Proficiency with Home Care Home Base EMR system (required)
• Must have a valid driver’s license (required)
• Prior travel experience is preferred but not required

Work Details

• Typical caseload: Approximately 7 patient visits per day
• Dress code: Matching scrubs
• Practice system: Point of Care
• Documentation and timely submission of hours are essential
