Experience Requirements

• Recent Home Health experience within the last 2 years
• Proficiency in OASIS documentation
• Wound care experience
• Excellent infection control practices in the home environment

Patient Population & Duties

• Home Health and Hospice patients including CHF, Diabetes, Renal Disease, Wound Care, IV therapy, management of drains and other devices
• Typical caseload: 6 patient visits per day (Admission plus 3-4 patients)
• Travel RNs are required to perform Start of Care (SOC), Re-certifications, and Resumptions
• No ventilator support provided for patients in Home Health
• No additional utilization review duties

Scheduling & Work Hours

• Monday to Friday, 8:00 AM to 4:30 PM
• Rotating weekends (typically every 4th or 5th weekend) with a day off before and after
• On-call rotation: one evening call per week and one weekend call per month
• Holiday work: typically up to two holidays per year

Other Information

• Nurses work both Home Health and Hospice; primary hospice patients typically not assigned to travel nurses but may see hospice patients on call with training provided
• Dress code: Any solid color, clean and professional
• CNA support available for Home Health/Hospice aides
