Job Description

  • Develops and oversees nursing programs related to patient care and clinical standards, policies, and procedures.
  • Develops and documents care plans and therapy programs.
  • Ensures implementation of plan of care and therapy programs, securing necessary resources and equipment.
  • May supervise and direct nursing staff.
  • Coordinates, assembles, and maintains required documentation.
  • Schedule: Monday-Friday, 8:00 AM - 4:30 PM

Health & Immunization Requirements

  • MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
  • Fit Test: Required for all travelers, preferred prior to start, updated annually
  • Hepatitis B: Documentation of vaccination series completion or signed declination
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
  • TB Screening: Positive IGRA or PPD results, negative chest X-ray within a year, and annual TB questionnaire
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) within 30 days of start
  • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
  • Tdap: Valid within 10 years, no declinations accepted
  • Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
  • Ascension 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually

Background & Verification Requirements

  • Ascension Profile (Resume or professional profile)
  • At least two professional references prior to first assignment
  • Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender, and other checks within 30 days of assignment
  • EPLS (Excluded Parties List System) check within 90 days of start
  • Verification of education relevant to clinical work within 90 days of start
  • Confirmation of Assignment documentation upon initial assignment and updates upon extension
  • Third-party employment verification for the past seven years

Additional Requirements

  • Ascension Skills Checklist (clinical skills competency) completed within one year of start and updated annually
  • Some optional checklists and evaluations may be required by the facility/unit
