Experience Requirements

Competency Exams
- Medication Test/Pharmacology Competency Exam: Required for all RN/LVN candidates prior to start.
- Unit-Specific Competency Exam: Required.
- Dysrhythmia/EKG Competency Exam: Required for all RNs within 1 year of start (except certain units); must be updated annually.
Background and Compliance Requirements
- Background Check: 7-year criminal background check within 90 days of start, including SS trace, counties lived/worked, nationwide, sex offender, MVR if driving.
- Fingerprinting: Ohio BCI & FBI fingerprinting required within 1 year of start using code 3701.881. New fingerprinting required if break in service >90 days or results older than 1 year.
- Pending Background Check Attestation: Required if fingerprint results are pending at start.
- Education Accreditation Confirmation: Must be graduate of an accredited nursing program (CCNE, ACEN, NLN CNEA, or CASN).
- Transcript/Diploma: Official transcripts or diploma required; third-party verification acceptable.
- Employment Application: Required for all candidates; nursing candidates must include education.
- References: Two professional references required; at least one from a manager or formal leadership.
Health and Immunization Requirements
- Physical Exam: Physician documentation within 1 year stating good health and free of contagious disease.
- Tetanus/Diphtheria/Tdap: Adult dose required after age 11/12; Tdap only.
- TB Testing: Negative PPD, QGold, or TSPOT within 1 year; TB questionnaire required annually.
- MMR (Rubeola, Mumps, Rubella): Two vaccines or positive titer required; boosters if low titer.
- Hepatitis B: Titer or 3-vaccine series; signed declination acceptable.
- Varicella: Two vaccines or positive titer required; medical exemptions reviewed.
- Drug Testing: Negative 10-panel drug screen within 3 months of start; new travelers cannot start with pending drug screen.
- Flu Vaccination: Required during flu season (Oct 1 - Mar 31) or declination with mask-wearing.
- COVID-19 Vaccination: Not required but strongly recommended; proof of primary vaccination collected if available.
Facility Policies and Additional Requirements
- Dress Code and Hand Hygiene Policy: Must be reviewed and signed for contracts starting 5/19/25 and beyond.
- Fraud, Waste, and Abuse Training: Certificate required prior to start and updated annually.
- Ethics and Compliance Policies: Must be reviewed and acknowledged annually for new contracts starting 11/7/24 and beyond.
- Confidential Fraud Waste Abuse and Compliance Reporting Form: Required for all candidates.
This position is a travel/contract role for a Registered Nurse in a medical-surgical/telemetry unit with a night shift schedule. Candidates must meet comprehensive certification, background, health, and compliance requirements to be eligible.
