Location: Sarasota, Florida
Employment Type: Travel/Contract
Contract Length: 20 weeks

Job Description & Responsibilities

  • Perform assessments and documentation
  • Skilled in cardiopulmonary/circulatory care
  • Use and knowledge of equipment and devices such as gait belts, rolling walkers, sit-to-stand lifts, sliding boards, collars, supplemental oxygen, prosthetics, wheelchairs
  • Orthopedic and neurological patient care
  • Develop and implement home exercise programs
  • Medication management
  • Patient and family education
  • Recognize signs and symptoms of physical, sexual, and psychological abuse
  • Safety and injury prevention
  • Billing knowledge
  • Home skilled visits: 6-8 visits per day (minimum 6 points per day, may increase)
  • Patient types include med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, respiratory patients, neuro, trachs, trauma, amputees (AKA, BKA), ADLs
  • Self-scheduled visits and hours based on office needs
  • 1-2 weekends per contract

Required Certifications & Licenses

  • BLS (Basic Life Support) - AHA or RQI accepted
  • State physical therapy license (valid and current)
  • OASIS training preferred
  • Must have valid driver’s license and reliable car for home visits
  • COVID-19 vaccination required or exemption on file
  • Flu shot required or declination with mandatory mask wearing at all times while at work

Equipment & Systems

  • Laptop, MiFi for internet access, portable printers provided
  • Use of BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system

Compliance & Documentation Requirements

  • Background check within 1 year including SS trace, county searches, nationwide, sex offender, education verification
  • Education verification required (transcript/diploma)
  • Foreign equivalency evaluation if education outside USA or Puerto Rico
  • Florida AHCA Level II fingerprinting clearance required prior to start
  • Physical exam within 6 months of start
  • Negative TB test or chest X-ray within 10 weeks of start (or within 1 year if prior positive PPD)
  • Annual TB questionnaire required
  • Immunizations: Measles, Mumps, Rubella, Hepatitis B, Varicella (titer or vaccines with boosters if needed)
  • Drug testing: Standard 9 panel within 30 days of start (instant tests not accepted)
  • Flu vaccination or declination with mask requirement
  • COVID-19 vaccination or approved exemption
  • Respirator medical clearance if respirator use required
  • Photo for badge prior to start
  • Personal or auto insurance required for home health positions
  • Dress code and tobacco policy compliance
  • Completion of BayCare Online Learning Center modules prior to start (4-8 hours)
  • References: 2 from past 2 years
  • Employment application and skills checklist required

  • Prior travel experience is not explicitly required for this position
  • Visits and hours are self-scheduled based on office needs
  • Must be able to work 1-2 weekends during the contract
  • Scrubs: Any solid color scrub
