Job Opportunity: Physical Therapist (PT) - Long Term Care (LTC)

Location: Snyder, Texas
Employment Type: Temporary
Contract Length: 13 weeks

Job Description

• Accurately assess and diagnose patient mobility and functional deficits.
• Develop individualized treatment plans with patient-centered goals to reduce pain, restore function, and prevent disability.
• Perform patient evaluations including history, systems review, and specific tests/measures.
• Establish and manage plans of care and treatment techniques per physician orders.
• Conduct ongoing reassessments and manage discharge planning from the first visit onward.
• Supervise and delegate tasks to ancillary staff and students in accordance with state laws and policies.
• Complete clear, accurate documentation within required timeframes and compliance standards.
• Communicate effectively with patients, families, caregivers, and healthcare team members using AIDET principles.
• Assess and provide education to patients and caregivers based on their needs.
• Participate in professional development and quality improvement activities.
• Maintain a safe environment for patients, visitors, and staff.
• Practice within the scope defined by Texas state law.

Requirements

• Experience: Minimum 2 years of Physical Therapy experience in a Long Term Care (LTC) setting.
• Licensing: Texas state license or compact license required.
• Preferred: Experience with Direction of Rehabilitation (DOR) highly preferred.
• Prior Travel Experience: Not explicitly required.
