Job Opportunity: Occupational Therapist (OT) - Acute Care Hospital

Location: Saint Cloud, Minnesota
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Work schedule: 5 days a week, 8-hour shifts (07:30 AM - 4:00 PM)
• Every 4th weekend required
• Must have previous NICU experience
• Must have an active license or have already applied (ETA 6 weeks)
• Minimum 1 year of experience in specialty required
• EPIC experience preferred

Required Experience

• Specialty: Therapy/Rehabilitation (Occupational Therapy)
• Minimum years of experience: 1 year (highest requirement found)
• Prior travel experience: Not explicitly required

Certifications & Licenses

• Must have a valid Occupational Therapy license (or have applied)
• ACLS, BLS, PALS, NRP, TNCC, EFM certifications: Not specifically required for OT but may be relevant depending on facility policies
• State License Primary Source Verification required

Background & Compliance Requirements

• Criminal background check within 1 year of start, including:

  • 7-year search
  • SS Trace
  • Counties
  • Sex offender registry
  • Nationwide check
  • Education verification (diploma acceptable)
  • International searches if candidate lived/worked outside the US
• Minnesota DHS NetStudy Fingerprint required (no start with fingerprints pending)
• EPLS/GSA/SAM and OIG excluded provider checks required
• Physical exam within 1 year of start
• Drug testing (9 panel) within 1 year of start (not required for client clearance but recommended)
• Immunizations and health documentation required:

  • Tdap vaccination (no religious declination accepted)
  • Negative TB test or chest X-ray per MN state guidelines
  • MMR (Measles, Mumps, Rubella) titers or vaccinations (time sensitive)
  • Hepatitis B vaccination, positive titer, or declination
  • Varicella titer or vaccination
  • Seasonal flu vaccination or declination (mask required if declined)

Facility Specific Requirements

• Candidate must complete MN DHS NetStudy Fingerprint Questionnaire immediately after signing contract
• Photo for badge required (grey background, no selfies, JPG format)
• Dress code must be reviewed prior to start
• Education modules (~16 hours) to be completed during orientation
• Confidentiality agreement must be signed prior to start
• Candidates must return badges on last day of assignment

• References: Two references from recent employers within 2 years required
• Skills checklist required for all clinical candidates
• Work history verification may be required for non-clinical roles (not applicable here)
