Location: Willmar, Minnesota

Job Description
- Position for a Speech Language Pathologist in a pediatric outpatient setting.
- Work schedule: Monday - Thursday, 9:00 AM to 5:30 PM; Friday, 7:30 AM to 4:00 PM.
- 5 days per week, 8 hours per day.
- Must live at least 25 miles from the facility.
- If called off by the facility, guaranteed contracted hours will still be paid.
Experience & Licensure Requirements
- Minimum 1 year of experience as a Speech Language Pathologist.
- Must hold a valid SLP license in the state of Minnesota.
- No explicit requirement for prior travel experience.
Certifications & Competencies
- Basic Life Support (BLS) certification required.
- Advanced certifications such as ACLS, PALS, NRP, TNCC, or EFM are not required for this pediatric SLP role.
- Competency exams and medication/pharmacology tests are not applicable for this role.
Background & Screening Requirements
- Criminal background check required within one year of start date, including:
- 7-year search
- SS Trace
- County checks
- Sex offender registry
- Nationwide check
- Education verification (diploma acceptable in lieu of third-party verification)
- International searches if candidate has lived/worked outside the US
- Minnesota DHS NetStudy Fingerprint clearance required (no start with fingerprints pending).
- GSA/SAM and OIG excluded provider checks required.
Health & Immunization Requirements
- Physical exam within 1 year of start.
- Fit mask (N95) required.
- Tdap vaccination required (no religious declination accepted).
- Negative TB test or chest X-ray per Minnesota state guidelines.
- MMR (Measles, Mumps, Rubella) proof required (positive titer or 2 vaccines).
- Varicella proof required (positive titer or 2 vaccines).
- Hepatitis B vaccination, positive titer, or declination accepted.
- Seasonal flu vaccination or declination required (mask required if declined).
- Drug testing not required for client clearance but must be within 1 year if done.
Additional Requirements
- Two references from recent employers within the last 2 years.
- Skills checklist required.
- Candidate must complete Minnesota DHS Net Study Questionnaire immediately after signing contract.
- Photo for badge required (grey background, no selfies or professional photos unless headshot).
- Dress code must be reviewed prior to start.
- Confidentiality agreement must be signed prior to start.
- Education modules will be completed during orientation (approx. 16 hours).

- Candidates cannot start without all required clearances and documentation.
- Background checks and fingerprinting must be completed and cleared before start.
- Candidates must comply with all facility-specific policies and procedures.
