Job Opportunity: Home Health Occupational Therapist

Location: Socorro, New Mexico

Job Summary

• Provide occupational therapy services in patients' homes, promoting self-management and caregiver involvement.
• Coordinate care with multiple disciplines to achieve discharge goals.
• Act as a patient advocate.
• Floating between departments and/or local locations within 20 miles is required.

Key Responsibilities

• Evaluate patients, develop and implement treatment plans to improve physical or developmental function and independence in daily activities.
• Continuously reassess treatment plans and goals.
• Instruct patients, families, and caregivers on therapy programs and use of adaptive equipment.
• Order and adapt equipment for safety and independence at home.
• Complete all required documentation daily, including admissions.
• Communicate with patients, families, physicians, and other healthcare providers regarding care.
• Maintain departmental policies, quality assurance, safety, and infection control standards.
• Participate in team conferences, educational programs, and special projects.
• Maintain productivity standards for visits and caseload management.
• Supervise Certified Occupational Therapy Assistants and instruct Home Health Aides/Certified Nursing Assistants in therapy follow-through.

Contract Length

• Approximately 25 weeks

Required Experience

• Minimum 1 year of supervised clinical practice in an acute or rehab setting preferred.
• Home Health experience preferred.

Education & Licensure

• Bachelor's degree and qualifications to be licensed as an Occupational Therapist in New Mexico.
• Must meet Home Health Medicare Conditions of Participation if applicable.
• BLS Certification required.
• Current driver's license and auto insurance required; driving assigned within specified regions.
