Position Details

Experience Requirements

• Physical/Health Statement: Documentation of fitness to work from exam within last year required.
• Drug Screen: 10-panel drug screen completed within 30 days of start, no THC allowed.

Professional & Compliance Requirements

• Background Checks: Federal, nationwide, county criminal background check within last 7 years; SSN verification.
• Education Verification: Highest academic degree or diploma/GED verified.
• Exclusion Lists: EPLS, GSA/OIG, NSO checks within 30 days of start.
• Confidentiality & Security Agreements: Must sign and have on file.
• Conflict of Interest Disclosure: Required.
• Code of Conduct Form: Required.
• I9 Verification: Standard I9 completed by agency.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Specialty Specific Competency Testing: Must be completed within the last year.
• Joint Commission Testing: CORE exam within the last year.
