Job Description
- 36 or 48 hour contracts acceptable
- Every other weekend required

Health & Immunizations
- MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
- Fit Test: Required annually (preferred prior to start)
- Hepatitis B: Documentation of vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
- TB Screening: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
- Tdap: Valid within 10 years, no declinations accepted
- Health Statement or physical exam prior to first assignment, updated annually
Background & Screening
- Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days of first assignment, updated annually)
- EPLS check within 90 days of start, rerun if break in service > 90 days
- 12 Panel Drug Screen within 30 days prior to start, includes MRO review for prescriptions or negative-dilute results
- Verification of education relevant to clinical work within 90 days of start
- Employment verification for past 7 years, updated annually or if break in service > 90 days
- Ascension Profile and at least two professional references required prior to first assignment
