Job Opportunity: RN - Home Health

Location: Tuscaloosa, Alabama
Employment Type: Home Health (Onsite)
Contract Length: 13 weeks


Job Description

The Home Health Visit Nurse provides skilled nursing care, education, treatment, and emotional support to patients in their homes. This role involves collaboration with the Home Health RN Care Manager and other disciplines to ensure patients attain and maintain their highest level of functioning. The nurse monitors patient progress, communicates changes to the care team, and revises care plans as needed.


Work Schedule

  • Hours: 8 am – 5 pm
  • Weekend Requirement: None
  • Holidays: As required
  • Patient Volume: 5-7 patients per day

Required Qualifications

Education

  • Associate Degree in Nursing

Licenses & Certifications

  • Registered Nurse license in Alabama or Compact license (No temporary licenses accepted)
  • AHA BLS or Red Cross certification
  • NURSYS registration
  • Valid driver’s license

Experience

  • Minimum 2 years nursing experience preferred
  • Will consider 18 months of experience under the highest level of professional license
  • Must have at least 1 year of experience in the specialty (Home Health Nursing)

EMR Experience

  • Proficient with Homecare Homebase (HCHB)

Essential Functions

  • Provide skilled nursing care to assigned patients
  • Monitor care provided by other disciplines (SN, PT, OT, ST, MSS, RD, HH Aide)
  • Educate patients and families on home health care strategies
  • Supervise patient and family in care and healthy environment maintenance
  • Evaluate and revise care plans collaboratively
  • Ensure physician orders are followed and goals are met
  • Perform skilled assessments and document findings accurately and timely (OASIS)
  • Measure wounds weekly and maintain accurate medication lists
  • Communicate regularly with the Home Health RN Care Manager
  • Establish positive relationships with physicians and office staff
  • Assess patient outcomes and suggest improvements
  • Participate in performance improvement plans and IDT meetings
  • Participate in on-call rotation

Physical Requirements

  • Ability to lift 50 pounds and assist patients over 200 pounds
  • Ability to stand, walk, kneel, sit, crouch, or stoop for extended periods
  • Ability to navigate uneven surfaces (fields, grass, gravel, concrete, etc.)
  • Good manual dexterity and communication skills
  • Tolerance for various environments including presence of pets and infestations
  • Hearing and vision normal or corrected to normal range
  • Physical presence onsite is essential
