Job Opportunity: Home Health/Hospice RN

Location: Bristol, Connecticut
Contract Length: 13 weeks

Job Details

• Discipline: Registered Nurse (RN)
• Specialty: Home Health/Hospice
• Shift: Monday to Friday, 8:00 AM – 4:30 PM (40-hour guarantee)
• Call Requirements: Every 4th weekend with a flexible day off during the week; weekend call involves callback within 20 minutes and patient visit within 30 minutes
• Scheduling: Self-scheduling available Monday to Friday; weekend rotation required
• Holiday Requirements: Rotate with permanent staff
• Patient Population: Adults and geriatrics; no pediatrics
• Common Diagnoses: Medication management, CFH, COPD, post-surgical care, orthopedic total joints, CVA, dementia, hospice care
• Patient Visits: Average 5-6 visits per day, using a weekly point system for visits
• Travel Requirements: Travel typically within a 20-minute radius in an urban area
• Orientation: 1 week with a preceptor
• Dress Code: Any clean and professional scrub color

Required Qualifications

• Licensure: Must be a licensed RN in Connecticut, fingerprinted as required by the state
• Certifications: BLS certification required
• Experience: Minimum 1 year of home care or hospice nursing experience (highest required years of experience)
• Preferred Skills: Experience with Meditech home care EMR and Oasis documentation highly preferred
• Other Requirements: Valid driver’s license, reliable car, and auto insurance

Additional Information

• No ventilator care required; oxygen therapy only (no vents, CPAP, BiPAP)
• Duties include patient evaluations, treatment, and plan of care development
• Home Health/Hospice aides are part of the care team
• Manager available Monday to Friday; administrator available 24/7
