Licensure

• Current and valid RN license for Oregon

Certifications (must be current and valid)

• BLS (American Heart Association certification required for all direct patient care personnel)
• ACLS
• PALS

Health and Safety Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (must be completed within 30 days of start, no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption except for CA or IL)
• Influenza Vaccine (proof or declination/exemption for 10/1/2024 - 4/30/2025 or 10/1/2025 - 4/30/2026)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (proof of 1st shot prior to start, subsequent shots per schedule; titers not accepted)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive (per regional requirements)
• Varicella (proof of vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)

Professional and Compliance Requirements

• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed)
• Confidentiality and System Security Agreement (signed)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission CORE Exam (within last year)
• NSO (National Sex Offender) check dated within 30 days, including aliases and locations
• System Access Form (completed and uploaded one week before start)
• Specialty Specific Competency Testing (copy of specialty exam completed within last year)
• Skills Checklist (provided by agency for clinical contractors)

Additional Notes

• Resume, skills checklist (within one year), and two professional references required for submission.
• License verification must be dated within 30 days of start.
• Drug screen panel includes cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, and methadone.
• Declinations for vaccines are not accepted in California or Illinois.
• All documentation must be uploaded to the credentialing system prior to onboarding.
