Job Description

• Provide direct and indirect family-centered therapy services aimed at increasing patients' functional abilities, independence, and community integration.
• Work as part of highly coordinated, multidisciplinary teams to serve patients with challenging and complex conditions.
• Deliver care organized around each patient’s condition, requiring exceptional teamwork, effective communication, and behaviors aligned with high-functioning integrated teams.

Responsibilities

• Conduct evaluations using principles of family-centered care.
• Design assessments based on evaluation goals, provider orders, and patient/family input.
• Obtain necessary patient history and background information.
• Select appropriate evaluation methods and tools tailored to the patient’s age and developmental, neurological, musculoskeletal, cardiopulmonary, cognitive, and behavioral status.
• Administer evaluations consistent with best practices and organizational competencies.
• Interpret evaluation results to patients and families, incorporating their feedback on reliability and validity.
• Make recommendations for treatment, referrals, equipment, resources, and home/community-based activities.
• Document and bill evaluations according to provided guidelines.
• Provide ongoing education, guidance, and support to patients, families, and the community.
• Collaborate with a wide variety of disciplines including medical professionals, community educators, interpreters, advocates, and payers.
• Exhibit excellent customer service skills.
