Job Opportunity: Occupational Therapist (OT) - Acute Care Hospital
Location: Lafayette, Indiana
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Work Monday through Friday plus one weekend day per month throughout the contract.
- Join an inpatient acute care rehab team of 19 professionals.
- Treat patients across all acuity levels, including ICU and progressive care units.
- Patient diagnoses include CABG and other cardiac procedures, stroke, TBI, joint replacement, fracture repairs, respiratory illnesses, dementia, MS, Parkinson’s, ambulation issues, etc.
- Responsibilities include evaluating patients, developing and following plans of care (POC), ADL training, therapeutic exercises and activities, and daily assessment of discharge disposition appropriateness.
- Charting system: Cerner.
- Scrub color: Royal Blue; Black shoes.
Required Qualifications
- Minimum of 1 year of Occupational Therapy experience in an acute hospital setting.
- Valid Indiana OT license.
- NBCOT certification.
- BLS certification (AHA or RQI accepted).
- Manual Therapy and Lymphedema certifications/licenses preferred but not required.
- Cerner experience preferred.

Required Certifications and Documents
- BLS (Basic Life Support) - AHA or RQI accepted.
- ACLS (Advanced Cardiac Life Support) - AHA or RQI accepted (verify per cost center).
- Other certifications such as PALS, NRP, CHEMO, TNCC, EFM, NIH Stroke Certification may be required depending on unit assignment.
- State License verification within 30 days before start.
- Background checks including criminal, SSN trace, sex offender registry, and education verification.
- Physical exam within 12 months of start date.
- Fit mask test within 12 months of start date (specific respirators approved).
- Immunizations and titers:
- Negative TB test or chest X-ray within 1 year.
- Rubeola (Measles), Mumps, Rubella, Varicella: Positive titer or 2 vaccines required; declinations not accepted.
- Hepatitis B series, titer, or signed declination required.
- Seasonal Flu vaccination/documentation required during flu season (9/30 - 3/31); declinations not accepted.
- Drug screening: Negative 10-panel drug screen within 12 months of start.
- Competency exams and medication/pharmacology test as required.
- References: Minimum 2 from recent employment within 2 years, with appropriate supervisory titles.
- Employment application with full work history and explanations for gaps over 30 days.
- Skills checklist within 12 months, updated annually.
Additional Notes
- Indiana RN travelers are restricted from starting new contracts after 9/9/2024 due to facility policy changes; this does not apply to Periop/OR RN’s, Allied, and Ambulatory RN travelers.
- On-site fit testing is limited at certain campuses; prior approval required for PAPR use.
- Facility-specific orientation and compliance documents must be completed as a priority.
