Job Opportunity: Speech Language Pathologist (SLP Outpatient)
Location: Cottonwood, Arizona
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Setting:
- 85% Pediatric Outpatient
- 15% Adult Outpatient
- 1 day Adult Acute Care
- Shift: 5x8 hour shifts
- Monday, Wednesday, Friday: Cottonwood (Adults/Pediatrics)
- Tuesday, Thursday: Sedona (Adults)
- Daily Patient Load: 12-14 patients
- Guaranteed Hours: 40 hours/week
- Facility may call off up to 6 times during the 13-week assignment
Required Qualifications
- License/Certification:
- Current CCC-SLP (Certificate of Clinical Competence in Speech-Language Pathology)
- BLS (Basic Life Support) - AHA only, AHA-RQI Provider Cards accepted
- Experience:
- Minimum 1 year of experience in Speech Language Pathology
- First-time travelers accepted (prior travel experience NOT required)
- Other Requirements:
- Valid Arizona state license for Speech Language Pathologist
- Current and valid fingerprint card issued by the State of Arizona required within 30 days of employment
- Background checks including 7-year history, SSN trace, criminal search, sex offender search, EPLS/GSA/SAM, and HHS/OIG within 30 days of start
- Completion of employment application with 3 years of work history and documented gaps if any
- Two professional references required (one from most recent employer within 1 year)
- Physical exam within 1 year of start, documenting ability to perform essential job functions
- Drug testing (10 panel) within 30 days of start and updated annually
- Immunizations and/or titers required:
- Tetanus/Diphtheria/Tdap (preferred within last 10 years or declination)
- Negative TB test or chest x-ray within 1 year and updated annually
- Measles (Rubeola), Mumps, Rubella: positive titer or proof of 2 MMR vaccines
- Hepatitis B: positive titer, 3-dose vaccine series, or declination
- Varicella: positive titer or proof of 2 vaccines
- COVID vaccination series or declination with facility-specific form
- Seasonal Flu vaccination or declination with facility-specific form (for candidates starting 09/29/2025 or later)
- Fit mask (N95) within 1 year of start and updated annually (PAPR not an option)
- Completion of competency exams and medication/pharmacology competency within 1 year of start, scoring 80% or higher
- Completion of facility-specific job description form prior to start
- Review and sign facility policies including:
- Security Question Form
- Travel Information Verification
- Workforce Member Confidentiality Agreement
- Substance Abuse Screening Policy
- Professional Attire and Appearance Policy
- Additional:
- Photo for badge required (clear background preferred)
- Color Blind Testing required for all candidates starting 1/19/2026 (100% passing score)

This position is ideal for Speech Language Pathologists with at least 1 year of experience who are interested in a pediatric and adult outpatient setting with some acute care exposure, working a consistent 5-day schedule in a teaching hospital environment.
