Job Opportunity: Physical Therapist (Rehabilitation Therapy)
Location: Grand Junction, Colorado
Employment Type: Contract

Job Description
Seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services. The role involves delivering skilled physical therapy in patients' homes following physician orders, payer requirements, agency policies, and state/federal regulations. The focus is on supporting patient mobility, safety, and functional independence through individualized, evidence-based care.
Clinical Responsibilities
- Perform physical therapy visits including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits
- Develop and implement individualized physical therapy plans of care
- Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention
- Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
- Communicate patient progress and changes to the interdisciplinary care team
- Collaborate in clinical oversight of Physical Therapist Assistants (PTAs) when applicable and permitted by Colorado law
Documentation & Systems
- Complete all required documentation accurately and timely using Homecare Homebase (HCHB)
- Ensure documentation complies with Medicare Conditions of Participation, payer requirements, and agency standards
- Maintain HIPAA compliance and privacy/security requirements
Care Coordination
- Collaborate with agency clinical staff including RNs, PTAs, OTs, STs, and leadership
- Participate in care coordination related to patient needs, visit completion, and regulatory requirements
- Provide clinical oversight of PTAs when applicable and permitted
Required Qualifications
- Current, unrestricted Colorado Physical Therapist license
- Graduation from an accredited Physical Therapy program
- Valid driver’s license and reliable transportation
- Ability to practice independently in a home-based setting
- Proficiency or willingness to learn Homecare Homebase (HCHB)
Preferred Qualifications
- Home health experience
- Familiarity with Medicare documentation and home health regulations
- Experience working with geriatric and medically complex populations
