Job Opportunity: Physical Therapist (PT) - Outpatient
Location: Kodiak, Alaska
Employment Type: Travel/Contract

Job Details

- Floating: Yes, within scope to cover outpatient, acute care, and some Skilled Nursing Facility (SNF) patients

- Locals Accepted: No (candidate’s permanent address must be at least 50 miles from facility)
- Pending License Accepted: No
- Guaranteed Hours: 72 hours per 2 weeks (may be called off for one 8-hour shift per 2 weeks)

Housing and Transportation
- Facility provides housing (typically 1-bedroom apartments) and a rental vehicle during the assignment
- Housing available no earlier than 72 hours before contract start and must be vacated within 24 hours after contract end
- Pets are not allowed
- Housing typically accommodates only the traveler; additional family members require approval
- Housing must be cleaned upon departure or additional cleaning fees apply
- Vehicle use limited to “around town” within a 30-mile radius; no off-road use
- Only the traveler may operate the vehicle; valid driver’s license and insurance proof required
- Vehicle must be returned clean and with a full tank of gas
- Tire chains available at additional cost
Required Qualifications
- Valid Physical Therapist license in Alaska
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification
- Experience working with pediatric patients
- Experience in outpatient, inpatient, and acute care settings
Experience and Skills
- Minimum 1 year of experience in Physical Therapy
- Ability to float within scope to cover multiple settings (outpatient, acute care, SNF)
- Prior travel experience is not required
Compliance and Documentation Requirements
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Drug screen (10 panel) within 90 days of start
- Immunizations and health documents including:
- TB test or chest X-ray within 12 months
- Tetanus/Diphtheria/Tdap within 10 years
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination (Alaska placements require positive titer and vaccine series)
- Varicella vaccination or positive titer
- Flu vaccine or declination during flu season
- COVID vaccine or declination
- Respirator clearance (RespSafety) required annually for travel candidates
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment application covering at least last 5 years of employment
- Skills checklist as applicable, updated annually
- Facility-specific modules and training via WorkRamp LMS, including:
- Basics of Trauma Informed Care (required at this facility)
- Specialty and regulatory modules as assigned
Additional Notes
- Candidates must comply with facility float policy and may be required to float to other Providence facilities within reasonable distance
- Facility uses Epic for charting; completion of Epic training modules may be required
- Housing and vehicle use policies are strict; adherence is mandatory
- This is a travel contract position with a critical access hospital serving a diverse patient population
