Job Opportunity: Home Health & Hospice RN

Location: Dickinson, North Dakota
Contract Length: 13 weeks

Job Summary

Seeking a Home Health & Hospice Registered Nurse to provide nursing care and case management for patients in a home health and hospice setting. The role involves initial and ongoing assessments, care planning, medication administration, patient and family education, and coordination with physicians and other healthcare providers.

Key Responsibilities

• Conduct initial and regular physical assessments and health history evaluations.
• Develop, initiate, and revise individualized care plans based on patient needs and diagnoses.
• Administer medications and treatments as prescribed.
• Provide healthcare instructions to patients and families.
• Identify and implement discharge planning needs.
• Serve as Case Manager when assigned, coordinating care for an assigned caseload.
• Prepare clinical notes and communicate with physicians regularly (at least every 60 days).
• Ensure necessary equipment and services are arranged.
• Supervise and evaluate home health aide care.
• Provide care within a 60-mile radius of Dickinson, including patients in homes and nursing facilities.
• Participate in on-call rotation for evenings, weekends, and holidays.

Schedule

• Monday to Friday, 8 AM to 5 PM
• On-call rotation for evenings, weekends, and holidays (24-hour call coverage on weekends/holidays)

Required Qualifications

• Current RN license in North Dakota.
• BLS (Basic Life Support) Certification.
• Home Health & Hospice nursing experience required.
• Valid Driver’s License and Auto Insurance.
• Ability to work independently with minimal clinical involvement.

Patient Population

• Adults, Geriatrics, Pediatrics in home health and hospice care settings.

Additional Information

• Scrub color: Any solid color.
• EMR system used: HomeCare HomeBase.
• Mileage is factored into compensation; no separate mileage reimbursement.
• Candidates must not have been employed full-time, part-time, or PRN with any facility associated with CommonSpirit (including CHI and Dignity) within the past year.
• Return to Office (RTO) required upfront; no additional RTO approvals after submission.

Experience Requirements

• Home Health & Hospice nursing experience required (years not explicitly stated, assume minimum 1 year).
