Job Opportunity: Home Health and Hospice RN
Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks
Job Summary
Provide skilled nursing, care, and case management for patients in their homes or at an in-patient Hospice Center. Responsibilities include admitting patients, coordinating services to ensure continuity of care, facilitating quality clinical and cost outcomes, and using the nursing process (assessment, planning, implementation, and evaluation) through an interdisciplinary team approach.
Essential Duties and Responsibilities
- Provide skilled nursing and coordinate care to patients and families in a residential setting based on physician authorization.
- Observe, assess, evaluate, and plan interventions related to patients’ physical and psychosocial status, including changes in condition or reactions to treatments/medications.
- Oversee care provided by Home Health/Hospice Aides and CNAs and evaluate compliance with the plan of care.
- Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines and community agencies.
- Provide quality care and symptom management to promote positive clinical outcomes and optimal quality of life through teaching, counseling, interventions, and referrals.
- Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements.
- Participate in team scheduling coordination and on-call rotation as needed, including patient assessments via telephone and home visits.
- Engage in quality improvement activities and attend in-service training.
Schedule
- Varies by setting:
- In-home RN: Monday to Friday, 8 AM to 5 PM
- In-patient RN: Rotating 3 shifts of 12 hours
Required Qualifications
- Current RN license in the State of Montana (BSN preferred)
- Minimum 2 years of recent acute care, home care, or hospice nursing experience
- Clinical skills preferred: venipuncture, catheterization, wound care, and/or parenteral therapy
- Valid driver’s license, appropriate auto insurance, and reliable transportation
