Job Opportunity: Home Care Registered Nurse

Location: Rutland, Vermont

Job Description

• Provide quality nursing care using the nursing process, adhering to evidence-based practice and state/federal regulations.
• Develop and communicate realistic plans of care involving patients and caregivers.
• Serve as Case Manager, managing caseloads and visit times per agency productivity standards.
• Complete OASIS documentation accurately and timely, staying current with documentation requirements.
• Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective patient care.
• Maintain and apply high-tech nursing skills such as IV use and maintenance, wound VAC, and complex wound care.
• Collaborate with LPNs within their scope of practice, delegate tasks, and review documentation.
• Evaluate and adjust plans of care based on patient responses and needs.
• Maintain accurate nursing documentation for effective healthcare team communication.
• Follow agency protocols for visit documentation and follow-up.
• Educate patients and caregivers on medication side effects, treatment options, discharge instructions, and reportable conditions.
• Identify and communicate interdisciplinary patient needs (e.g., physical therapy, social work).
• Complete required agency, state, and federal forms timely (e.g., HHABN, Medicare Notice of Non-Coverage).
• Manage visits efficiently and communicate scheduling needs promptly.
• Assess patient needs for Home Care Aide services and supervise accordingly.
• Report patient incidents to Quality Improvement specialists as needed.
• Coordinate safe patient discharges/transfers with appropriate documentation.
• Collaborate with physicians, providers, team members, and vendors to support care plans.
• Participate in monthly peer review audits and case conferences to enhance communication and care quality.
• Advocate proactively for patient care sensitive to individual patient/family needs.
• Participate actively in interdisciplinary team (IDT) meetings to develop and update individualized care plans.
• Stay current on pain control, symptom management, and infection control standards.

Required Qualifications

• Specialty: Nursing (Home Care Registered Nurse)
• Experience: Minimum 1 year nursing experience (default higher if discrepancy found; here only one value present)
• Licensure: Current Registered Nurse (RN) license valid in Vermont
• Skills:

  • OASIS experience required
  • Proficiency in IV therapy, wound VAC, complex wound care
  • Strong documentation and case management skills
  • Ability to educate patients and caregivers effectively
  • Ability to collaborate with interdisciplinary teams
