Job Opportunity: Physical Therapist (PT) - Home Health

Location: Lake Zurich, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks (with option to extend if mutually agreeable)

Job Description

• Provide physical therapy services in a home health setting, including home visits and periodic office meetings.
• Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (most assignments are standard 5x8s; some may have a 4x10 Baylor shift schedule Friday-Monday).
• Weekend rotation: Approximately 1 Saturday per month for admission needs.
• On-call: Not required for this position.
• Territory: Must be willing to travel up to 75 miles from home clinic, averaging 200+ miles per week.
• EMR system used: HCHB.
• Local candidates accepted.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health experience (first-timers not accepted).
• Current Physical Therapist license in hand (pending licenses not accepted unless only a few days to obtain).
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and current driver's license required.
• Agency-required liability insurance required.

Additional Requirements

• Must be willing to cover the southern territory (Evanston to Hampshire area).
• Must meet productivity expectations of 28-32 weekly units.
• Background checks, drug screening, and various state-specific clearances required prior to start.
• Must complete required compliance, confidentiality, and policy training and documentation prior to assignment start.
• Physical exam and immunization documentation (including TB, MMR, Hepatitis B, Varicella, Flu, COVID) must be current and on file.
• References: Two references required for all placements.
• Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.

Notes

• Guaranteed hours are not provided; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.
• Weekend and on-call requirements may vary by location; discuss during interview.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not billable).
