Job Opportunity: Physical Therapist (PT) - Home Health
Location: Dunedin, Florida
Employment Type: Travel/Contract
Contract Length: 20 weeks
Job Description
- Provide home health physical therapy services with a focus on cardiopulmonary, geriatrics, neuro, ortho, and oncology patient populations.
- Conduct home skilled visits, typically 6-8 visits per day (minimum 6 points per day, adjustable by manager).
- Responsibilities include assessment, documentation, use of equipment and devices (crutches, gait belt, hospital bed, rolling walker, slings, sliding boards, collars, supplemental oxygen delivery), ortho and neuro therapy, home exercise programs, medication education, patient and family education, and safety/injury prevention.
- Must be able to self-schedule visits and hours based on office needs.
- Every 3rd weekend work required.
- Use of laptop, MiFi for internet access, and portable printers for documentation.
Required Qualifications
- Discipline: Physical Therapist (PT)
- Specialty: Therapy/Rehabilitation
- Experience: Minimum 2 years of home health experience required.
- Licenses/Certifications:
- Valid state Physical Therapist license.
- BLS (Basic Life Support) certification required.
- OASIS training preferred.
- Other Requirements:
- Valid driver’s license and reliable car for home visits.
- COVID-19 vaccination required or approved exemption on file.
- Flu shot required or declination on file; mandatory mask wearing at all times while working.
- Scrubs: Any solid color scrub.

Compliance and Documentation Requirements
- Background checks including 7-year history, education verification, EPLS/GSA/SAM, HHS/OIG checks.
- Foreign equivalency evaluation if education completed outside the US or Puerto Rico.
- Fingerprinting: Florida AHCA Level II fingerprint clearance required prior to start; must be current and on file.
- Physical exam within 6 months of start date.
- Negative TB test or chest X-ray within 10 weeks of start (or within 1 year if past positive PPD).
- Annual TB questionnaire required.
- Immunizations or titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, series, or declination)
- Varicella
- Drug testing: Standard 9-panel within 30 days of start (instant tests not accepted).
- Seasonal flu vaccination or declination with mask requirement.
- COVID-19 vaccination or approved exemption.
- Respirator medical clearance if respirator use is required.
- Photo for badge (professional, clear, solid background).
- Personal or auto insurance current and active for home health positions.
- BayCare Health MSP dress code and tobacco policy compliance.
- Completion of BayCare Online Learning Center modules prior to start (4-8 hours).
Competency Requirements
- Medication/Pharmacology competency exam (score 80% or higher).
- Home Health RN competency exam (applicable to PTs working in home health setting).
- Color blind testing (Relias Color Blind Exam) prior to start.
- Dysrhythmia/EKG competency exam not required for PT.
- Stroke education CEUs and related certifications not required for PT.
- Crisis Prevention Institute (CPI) not required for PT.
Additional Notes
- Must have at least 2 references from the past 2 years.
- Employment application and skills checklist required.
- Must be able to work independently in a home health environment.
- Self-scheduling flexibility required.
- Every 3rd weekend work is mandatory.
