Job Opportunity: Hospice Registered Nurse
Location: Bennington, Vermont
Employment Type: Contract
Contract Length: 13 weeks
Job Description
- Provide occasional Home Care visits.
- Utilize the nursing process to assess patient and caregiver needs and deliver quality care in compliance with evidence-based practice, State and Federal regulations, and Vermont State Board of Nursing standards.
- Develop and communicate realistic plans of care involving patients and caregivers.
- Serve as Case Manager for assigned patients, managing caseloads and visit times per agency productivity standards.
- Complete OASIS documentation accurately and timely, staying current with documentation requirements.
- Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective, quality care.
- Maintain proficiency in high-tech skills such as IV use and maintenance, wound VAC, and complex wound care, ensuring infection control standards.
- Collaborate with LPNs within their scope of practice, delegate tasks, review documentation, and act on recommendations.
- Evaluate and adjust plans of care based on patient responses and identified problems.
- Maintain accurate nursing care documentation for effective communication within the healthcare team.
- Follow agency protocols for visit documentation and follow-up.
- Educate patients and caregivers on medication side effects, treatment options, discharge instructions, and reportable conditions.
- Identify and communicate interdisciplinary patient needs (e.g., physical therapy, social work) promptly.
- Complete required Agency, State, and Federal forms timely, including HHABN and Medicare Notice of Non-Coverage.
- Manage visits efficiently and communicate scheduling needs proactively.
- Assess patient needs for Home Care Aide services and coordinate plans of care accordingly.
- Report patient incidents to Quality Improvement specialists as needed.
- Coordinate safe patient discharges and transfers with appropriate documentation.
- Collaborate with physicians, providers, team members, and vendors to support the plan of care.
- Participate in monthly peer review audits and provide/accept constructive feedback professionally.
- Schedule and facilitate case conferences to enhance team communication and document meetings.
- Advocate proactively for patient care issues sensitive to individual patient and family needs.
- Participate actively in interdisciplinary team (IDT) meetings to develop and update individualized plans of care.
- Stay current on pain control, symptom management, and advanced nursing skills.
Required Qualifications
- Licensed Registered Nurse (RN) in Vermont.
- Experience in hospice or home health nursing preferred.
- Ability to perform comprehensive patient assessments and case management.
- Proficiency in high-tech nursing skills including IV therapy and wound care.
- Strong documentation skills and knowledge of OASIS requirements.
- Effective communication and interdisciplinary collaboration skills.
