Job Opportunity: Physical Therapist - Home Health

Location: Sunnyvale, California (Territory includes San Francisco, Brisbane, Daly City, Colma, South SF, Pacifica, Millbrae, Hillsborough, Burlingame, and neighboring cities)
Employment Type: Contract
Contract Length: 13 weeks

Job Summary

Restore function to prevent or minimize disability following disease or injury
Help clients reach maximum performance/function within their capabilities
Evaluate and treat patients using medically prescribed physical therapy programs
May supervise Physical Therapy Assistants
Conduct initial admission/evaluation visits
Notify Clinical Team Manager of OASIS scores within established timelines
Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans
Perform other duties consistent with skills and training aligned with organizational goals

Required Qualifications

Specialty: Rehabilitation Therapy (Physical Therapist)
Minimum 1 year of Home Health experience
Valid Physical Therapist license for California
