Description
Job Opportunity: PT Home Health (Therapy/Rehabilitation)
Location: Pittsfield, Massachusetts
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 13 weeks)
Job Description
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average patient load: 5-6 patients per day (5-10 patients assigned daily, equating to roughly 6-8 points per day).
- Types of cases: Start of Care and Follow Ups.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Weekend rotation: None.
- On-call: None.
- Territory coverage: Must be willing to travel up to 75 miles from home clinic, approximately 50 miles per week.
- EMR system used: HCHB.
- Local candidates accepted at the same rate.
Required Qualifications
- Minimum 1 full year of recent Adult Home Health experience (first-timers not accepted).
- Current Physical Therapy license valid in Massachusetts (license must be in hand; pending licenses accepted only if they can be obtained within a few days).
- CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
- Reliable transportation and current driver’s license.
- Agency-required liability insurance.
Additional Requirements
- Must complete all required background checks, including but not limited to:
- Standard 7-year background check with SS trace, county, nationwide, sex offender, and state-specific searches.
- State-mandated releases and authorizations (e.g., Massachusetts iCORI).
- Drug screening (10-panel).
- Education verification.
- Health documents required prior to start:
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Flu vaccination or documented declination for current season.
- COVID vaccination series or documented declination.
- Must review and sign facility-specific compliance documents, including but not limited to:
- Code of Conduct, Confidentiality Forms, HIPAA and Privacy Policies, Compliance Training, and Home Health Scenarios.
- Must be willing to comply with mileage reimbursement policies (patient-to-patient travel reimbursed; commute to/from first and last patient not reimbursed).
Notes
- Weekend and on-call requirements vary by location and should be discussed during the interview.
- Contract is a vacated position due to workload volume in the Northern Region of the territory.
- Candidates must be prepared to complete all required documentation and compliance training prior to start.
- Prior travel experience is not required for this position.
