Job Description

Join a dedicated home health team providing compassionate, one-on-one, in-home care focused on promoting strength, mobility, and independence. This role offers the opportunity to make a lasting impact through personalized therapy plans and collaborative care.

Key Responsibilities

Evaluate patients and develop personalized therapy plans in the home setting
Deliver evidence-based interventions to restore mobility and function
Educate patients and families on exercises and safety strategies
Collaborate with interdisciplinary teams for coordinated, goal-oriented care
Maintain accurate and timely documentation to meet quality and compliance standards
Adapt care plans based on patient progress and physician feedback
Support patients through discharge planning and wellness strategies

Required Qualifications

Licensed Physical Therapist (PT) in the state of New Mexico
Minimum 1 year of clinical experience (home health experience preferred)
Skilled in patient communication, documentation, and problem-solving
Self-motivated and comfortable working independently
Proficient with electronic health records and mobile charting
Passionate about function-focused, relationship-driven care

Cons

Generally speaking, Host Healthcare doesn't always have the highest rate on the market. Having gone with competing agencies in the past, I have often been disappointed and unsupported and ultimately found it wasn't worth the small pay bump
