Job Opportunity: Home Health Occupational Therapist

Location: Albuquerque, New Mexico
Employment Type: Contract

Job Description

Join as a Home Health Occupational Therapist providing in-home care to patients recovering from illness, injury, or hospitalization. Empower patients to regain independence and improve daily function through personalized therapy interventions.

Key Responsibilities

  • Perform in-home assessments to identify functional impairments and therapy needs
  • Develop and implement individualized plans of care focusing on safety, self-care, and mobility
  • Provide hands-on therapy interventions targeting activities of daily living (ADLs), home safety, and patient-specific goals
  • Educate patients and caregivers on adaptive equipment and techniques
  • Collaborate with physical therapists, nurses, aides, and physicians for coordinated care
  • Document patient progress and adjust care plans as needed
  • Support safe patient transitions and promote long-term functional wellness

Required Qualifications

  • Licensed Occupational Therapist (OTR/L) in New Mexico
  • Minimum of 1 year of clinical experience (home health experience preferred)
  • Strong problem-solving, communication, and time management skills
  • Ability to work independently in patients’ homes
  • Experience with EMR systems and mobile charting tools
  • Passion for enabling patient independence and dignity through skilled therapy

Generally speaking, Host Healthcare doesn't always have the highest rate on the market. Having gone with competing agencies in the past, I have often been disappointed and unsupported and ultimately found it wasn't worth the small pay bump
