Description
Job Opportunity: Radiology Technician
Location: Pontiac, Michigan
Employment Type: Travel
Contract Length: 12 weeks
Work Schedule
- 4-10 hour shifts per week, including every other weekend (EOW)
Required Qualifications
- Valid Radiology Technician license for Michigan (license verification must be dated within 30 days of start)
- American Heart Association BLS certification (attach front and back of card with expiration date)
- Specialty-specific competency testing completed within the last year
- Physical/Health Statement confirming fitness to work (exam within the last year)
- COVID-19 vaccination proof or approved declination/exemption (declination not accepted for CA or IL)
- MMR vaccination proof or primary source IgG titer lab report
- Fit Test (specific to Iowa: 1860 mask only)
- Hepatitis B vaccination series proof (titers not accepted)
- Influenza vaccine proof or declination for current and next flu seasons (declination not accepted for CA or IL)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) test (positive TB requires chest x-ray within last year for Michigan)
- Varicella vaccination proof or primary source IgG titer lab report
- 10-panel drug screen completed within 30 days of start (NO THC)
- Tdap vaccine (declinations accepted if no pediatric interaction)
- Respirator evaluation with physician signature (required for certain locations)
Professional Requirements
- Resume and skills checklist within the last year
- Two professional references
- Confidentiality and proprietary information agreement signed
- Conflict of interest disclosure form signed
- Criminal background check (federal, nationwide, county from last 7 years, including SSN verification)
- Education verification (highest level of academic education)
- EPLS (Excluded Parties List System) check within 30 days
- GSA/OIG exclusion check within 30 days
- Integrity and audit services form signed prior to start
- Joint Commission testing (CORE exam within the last year)
- National Sex Offender Registry check within 30 days (including statewide for Michigan)
- System access form completed and uploaded one week before start
- Code of conduct form signed
- I9 form completed by agency
