Job Opportunity: Physical Therapist - Outpatient

Location: Decatur, Illinois

Position Details

• Specialty: Therapy/Rehabilitation (Physical Therapist - Outpatient)
• Shift: 5 days a week, 8-hour shifts (8:00 AM - 4:30 PM); 4x10s schedule may be available
• Weekend Requirement: None, unless desired
• On-call: Not required
• Floating: May float within scope to meet facility needs
• Locals Accepted: Yes, no radius restrictions
• Return to Office Restrictions: Ideally no more than 5 days

Required Qualifications

• Valid Physical Therapist license in Illinois
• Minimum 1 year of experience as a Physical Therapist, with experience within the last year
• BLS certification required

Certifications (may be required based on unit/specialty)

• BLS (Basic Life Support) - AHA (RQI acceptable)
• ACLS (Advanced Cardiac Life Support) - if applicable
• PALS (Pediatric Advanced Life Support) - if applicable

Background and Compliance Requirements

• Valid/current US Driver’s License or US ID
• Background check within 30 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
• Education verification (degree verification required)
• Competency exam required for all clinical candidates
• Photo for badge required within one week of signing contract
• Code of Conduct acknowledgment required
• Physical exam within one year of start, including labs within 7 days of contract signing
• Fit mask test required (specific mask types accepted)
• Negative TB test or chest X-ray within one year
• TB questionnaire updated annually
• Immunizations or proof of immunity (MMR, Hepatitis B, Varicella) or signed declinations
• 9-panel drug screen within 30 days of start
• Seasonal flu vaccination or approved medical/religious exemption required annually

State-Specific Requirements (Illinois)

• Illinois Department of Corrections inmate search within 30 days of start
• Illinois Most Wanted search within 30 days of start
• Illinois State Police Murderer and Violent Offender Against Youth Registry search within 30 days of start
• Illinois Healthcare Worker Registry fingerprint results required for clinical candidates without an IL state license; fingerprints must be completed in Illinois 3 business days prior to start
