Physical Therapist - Home Health (Travel/Contract)

Location: Valrico, Florida

Job Summary

Seeking a Physical Therapist for a travel contract in home health. The role involves home visits and occasional visits to care facilities. The schedule is typically Monday through Friday, 8 AM to 5 PM, with occasional weekend call approximately every 5-6 weeks.

Contract Length

• 13 weeks

Patient Load & Productivity

• Average 5-6 patients per day
• Expected weekly productivity: 30 units
• Daily travel between 30-100 miles

Job Details

• Types of cases: Start of Care, Follow Ups
• Visits primarily in patient homes, occasional facility visits
• Must be willing to travel up to 75 miles from home clinic
• Mileage reimbursed at IRS standard rate (patient home to patient home)
• Guaranteed hours: No, but options to float to nearby clinics if patient census is low
• EMR systems used: HomeCare HomeBase, Oasis, Outlook, Teams

Required Qualifications

• Valid Physical Therapist license in Florida
• Minimum 1 year of Physical Therapist experience within the last year
• Experience working with adult patients in a Home Health setting
• BLS certification

Additional Information

• On-call: Occasional, approximately every 5-6 weeks
• Weekend requirements vary; discuss during interview
• Local candidates accepted at the same rate
• Position may convert to permanent after contract
