Job Description

• Care for Pulmonary & Medical Overflow patients
• Unit size: 22 beds
• Patient ratio: 1:4-5

Required Qualifications

• Valid Registered Nurse (RN) license in Indiana
• Minimum of 2 years of Medical/Surgical nursing experience
• Medical/Surgical nursing experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Onboarding & Compliance Requirements

• Signed assignment agreement prior to start
• Completion of pharmacology and medical-surgical exams with minimum 80% score (max 2 attempts)
• Completion of EKG exam with minimum 80% score (updated version required)
• Completion of core competencies including HIPAA, Joint Commission, NPSG, OSHA topics (minimum 80% score)
• Color vision test
• Health statement or physical exam prior to first assignment, updated annually
• Immunizations and health screenings:

  • MMR (two vaccines or positive titer; no declinations accepted)
  • Varicella (two vaccines, serological evidence, or verified history; no declinations accepted)
  • Tdap (valid within 10 years; no declinations accepted)
  • Influenza vaccine during flu season (exemptions accepted with mask requirement)
  • Hepatitis B vaccination series or signed declination
  • TB screening: 2-step PPD or IGRA within required timeframes; chest X-ray if positive; annual TB questionnaire
• Fit test for respiratory protection (preferred before start, can be done onsite)
• 12-panel drug screen within 30 days prior to start, updated annually
• Criminal background check including Social Security trace, OIG search, National Sex Offender registry, and others within 30 days prior to start
• Employment and education verifications within 90 days prior to start
• Professional references (at least two) prior to first assignment
• Driver’s license or state ID (valid, matching address requirements)
• Onsite onboarding including nursing contingent worker onboarding and education attestation
• Badge photo (forward-facing headshot with white or blue background, no alterations)

Additional Notes

• Optional unit-specific checklist may be required by unit leader within one week of start
• Various documents and verifications must be updated annually or upon contract extension
• Compliance with all facility-specific policies and procedures is mandatory
