Physical Therapist (PT) - Travel Assignment

Location: Crown Point, Indiana
Contract Length: 13 weeks
Employment Type: Travel

Facility Setting

• Freestanding 40-bed Inpatient Rehabilitation Facility (IRF)
• May be required to float to other units as needed (specifically to an Acute Rehabilitation Unit in a nearby location)

Work Schedule

• Standard hours: 8:00 AM – 4:30 PM
• If contract is renewed: 1 Saturday coverage per month, if able

Specialty & Experience

• Specialty: Physical Therapy (PT)
• Required Years of Experience: 2 years (acute rehab experience preferred, but strong applicants with less experience will be considered)

Electronic Medical Record (EMR) System

• EPIC

Additional Information

• Reason for opening: Replacement of permanent staff
• Dress code: Caribbean Blue scrubs
• Parking: Employee parking lot at the rear of the building

Required Qualifications

• Valid Physical Therapy Assistant license in Indiana
• Minimum of 1 year of Physical Therapy Assistant experience, including experience within the last year
