Registered Nurse - Transitional Care (LTC/SNF)
Location: Portland, Maine

Job Details
- Specialty: Transitional Care – Long Term Care / Skilled Nursing Facility (LTC/SNF)
- Shift: Nights
- Weekly Hours: Preferred 48 hours, minimum 36 hours acceptable
- Weekend Hours: Every other weekend, 36 hours
- Call Requirements: None
- Contract Length: 13 weeks
Unit Information
- Number of Beds: 46
- Patient Ratios: Maximum 1:8
- Patient Population: Mix of medical and surgical patients, medically ready for discharge
- Equipment Used: Specialty beds, Philips vital sign monitors, overhead lifts, standing scale, bladder scanner, various specialty beds and mobility aids
- Electronic Medical Record (EMR): Epic
Required Qualifications
- Valid Registered Nurse (RN) license in Maine
- Minimum 1 year of acute care experience within the last year
- Basic Life Support (BLS) certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

- Float assignments may be required based on skillset
- Patient-centered care approach, teamwork, excellent communication, punctuality, and reliability are important personality traits
- Dress code: Scrubs of any color, fragrance-free
- Unacceptable attire includes artificial nails, hoodies, graphic t-shirts, open-toed shoes, and any unprofessional dress or grooming
