Physical Therapist - Contract Opportunity

Location: San Ramon, California

Contract Length: 13 weeks

Job Description

Seeking a qualified Physical Therapist to provide rehabilitation therapy services. The assigned territory includes multiple areas within the San Jose region and surrounding cities. Additional travel outside the assigned area and weekend coverage may be required to maintain weekly visit expectations.

• Must be willing to travel outside the service area occasionally.
• May be required to see COVID-positive and COVID PUI patients.
• Productivity expectations include completing a minimum number of patient visits per week.
• Hours approved on a per-visit basis, covering drive/travel time, visit time, documentation, and communication with patients and doctors.

Required Qualifications

• Current California Physical Therapist (PT) License
• Basic Life Support (BLS) certification
• Valid Driver’s License and reliable transportation
• Up-to-date immunizations and screenings including:

  • TB (annual)
  • MMR
  • Varicella
  • Hepatitis B
  • Tdap (within 10 years)
  • Flu (annual)
• Drug screening and physical exam clearance

• All requested time off must be disclosed at the time of submission; time off requests after acceptance will not be approved.
• Compliance with agency contract terms and assignment dates is mandatory.
• Weekend and travel flexibility required to meet caseload and productivity standards.
