Job Opportunity: Radiation Therapist

Location: Davenport, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Required Qualifications

• Valid Radiation Therapist license in Iowa
• Minimum of 1 year of Radiation Therapist experience within the last year
• ARRT (American Registry of Radiologic Technologists) certification

Required Credentials and Documentation (Must be current and uploaded prior to or at onboarding)

• Professional license verification dated within 30 days of start
• Specialty-specific competency testing completed within the last year
• Fit test
• 10-panel drug screen completed within 30 days of start (including specific substances, no THC)
• COVID-19 vaccine proof (including booster by Sept 2025), or approved declination/exemption (declination not accepted in CA or IL)
• MMR vaccination proof (2 doses) or IgG titer lab results
• Hepatitis B vaccination series started prior to start date (titers not accepted)
• Influenza vaccine proof or declination/exemption for current flu season (declination not accepted in CA or IL)
• 2-step PPD or IGRA TB test (with chest x-ray if positive, per regional requirements)
• Varicella vaccination proof or IgG titer lab results
• Physical/health statement confirming fitness to work within the last year
• Tdap vaccine proof or declination (declination accepted if no pediatric interaction)

Additional Professional Requirements

• Criminal background check (federal, nationwide, county) covering last 7 years, dated within 30 days of start
• Education verification (highest degree or diploma/GED)
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days, including aliases and statewide checks for Iowa
• Conflict of Interest Disclosure form
• Code of Conduct form
• Confidentiality and Information Security Agreement
• Integrity and Audit Services form
• Non-Employee Upload form (pages 1-3 and 28)
• Joint Commission CORE exam within the last year
• System Access form completed and uploaded one week before start
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries) within 30 days prior to start
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days
