Job Opportunity: Home Health Physical Therapist

Location: Newport, Vermont
Employment Type: Contract
Contract Length: 13 weeks

Job Description

Provide outpatient home health physical therapy services.
Must have experience with OASIS, home health, and hospice care.
Ability to travel from patient to patient for home visits.
Must be able to manage and adjust to changes in schedule.
Work schedule is Monday through Friday, 8 AM to 5 PM, with holiday coverage as needed.
Use of an electronic medical record (EMR) system is required.

Required Qualifications

Valid Physical Therapist license in Vermont
Physical Therapist with at least 2 years of experience, including experience within the last year
Experience in a Home Health setting
Experience with OASIS
Experience in hospice care
Experience traveling patient to patient for home visits

Additional Information

Prior travel experience is not explicitly required.
Specialty experience required: 2 years in Rehabilitation Therapy.
