Job Description

Seeking a Registered Nurse with skilled home care experience (private duty nursing experience alone is not accepted). The role involves providing skilled nursing care in patients' homes, including:

• OASIS training and experience required
• IV skills and wound care
• Care for general medical diagnoses and high-risk diagnoses such as heart failure, COPD, myocardial infarction, pneumonia, and stroke
• Post-operative care
• Cardiopulmonary assessment, basic wound care, ABI compression dressing, wound vac dressing
• Care involving Pleurx/Aseptdrain, Foley catheter changes, staple/suture removal
• Use of glucometer, pro-time machine, SQ/IM injections, lab draw venipuncture
• PICC line or Midline care and dressing changes, SQ port access/deaccess
• Approximately 6-8 home skilled visits per day
• Flexible day/shift hours Monday through Friday, with some weekend shifts and alternating schedules

Required Qualifications

• Valid Registered Nurse (RN) license in Florida
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• Minimum 1 year of skilled home health nursing experience within the last year (skilled home care experience required; private duty nursing experience alone is not accepted)
• Experience with OASIS
• IV skills
• Experience caring for high-risk diagnoses including heart failure, COPD, myocardial infarction, pneumonia, and stroke
• Skilled in SQ port access and deaccess
• Basic Life Support (BLS) certification issued by the American Heart Association
• Advanced Cardiac Life Support (ACLS) certification

Required Documentation & Compliance

• Background checks including education verification, criminal history, and federal exclusions
• Physical exam within 6 months of start
• Negative TB test or chest X-ray within 10 weeks of start (or within 1 year if prior positive TB)
• Immunizations or titers for Measles, Mumps, Rubella, Varicella, Hepatitis B
• Drug screening (standard 9 panel) within 30 days of start
• Annual TB questionnaire
• Respirator medical clearance if applicable
• Photo for badge (professional, clear, solid background)
• Personal or auto insurance current throughout assignment
• Completion of required online training modules prior to start
• References from the past 2 years
• Employment application and skills checklist

Additional Information

• Scrubs: Any solid color scrub
• Laptop, MiFi, and portable printers provided for documentation
• Documentation systems include electronic health records and scheduling software
• Stroke education CEUs and other competency exams may be required depending on assignment specifics
