Job Opportunity: Outpatient Physical Therapist

Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (LTA)

Job Description

• Provide patient care for individuals requiring rehabilitation in an outpatient setting.
• Focus on optimizing patients' physical, functional, psychological, social, and emotional well-being.
• Schedule:

  • Monday, Wednesday: 9:30 AM – 6:00 PM
  • Tuesday, Thursday, Friday: 8:30 AM – 5:00 PM
• Outpatient orthopedic experience is required.

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Experience in an outpatient setting
• Outpatient orthopedic experience

Onboarding & Compliance Requirements

• Submission of a professional color photo (recent, plain background, no filters or eyeglasses).
• Verification of professional license and certifications (primary source verification required).
• Documentation of immunizations and health screenings including:

  • MMR (measles, mumps, rubella) vaccination or immunity proof
  • TB test within 3 months of start date (skin test or blood test)
  • Health statement signed by a licensed provider within 1 year
• Criminal background checks including:

  • Massachusetts CORI check
  • National sex offender registry check
  • Exclusion checks (OIG, GSA, Medicaid lists)
• Submission of resume covering past 7 years of work experience with city and state details.
• Two professional references within the last 4 years (at least one supervisor or higher).
• Completion of a skills checklist within 1 year of submission.
• Confirmation of assignment form upon acceptance and for any start date changes.
• Education verification through background check company.
• Social Security Number trace as part of background check.

Additional Preferred/Optional Health Requirements

• Hepatitis B vaccination or immunity proof (strongly preferred)
• Tdap vaccination within the last 10 years (preferred)
• If positive TB test, documentation of a negative chest x-ray is required.
