RN Case Manager - Home Health/Hospice

Location: Lebanon, New Hampshire (with service areas in surrounding VT and NH towns)
Employment Type: Contract
Contract Length: 16 weeks


Job Description

As an RN Case Manager in Home Health and Hospice, you will:

  • Screen, evaluate, re-evaluate, and treat patients in their homes.
  • Coordinate and communicate treatment activities with an interdisciplinary care team.
  • Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
  • Provide ongoing assessment of patients’ clinical, functional, and emotional status.
  • Communicate significant changes to physicians and team members.
  • Document evaluations, treatment goals, and objectives.
  • Teach patients and caregivers self-care techniques, medication, diet, and provide health counseling as appropriate.

This role offers broad independence while being part of a cohesive patient-care team, with a focus on work/life balance and flexibility.


Required Qualifications

  • Valid RN license in New Hampshire
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum 2 years of Home Health experience, including experience within the last year
  • Experience in Hospice care
  • Current ACLS certification
  • Current BLS certification
