Job Opportunity: Physical Therapist - Home Health

Location: Roseburg, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide physical therapy services in a home health setting with home visits and periodic office meetings.
  • Average caseload: 5-6 patients per day.
  • Types of cases: Start of Care and Follow Ups.
  • Must be willing to travel up to 75 miles from the home clinic, averaging about 100 miles per week.
  • Schedule typically Monday through Friday, with some positions offering a 4x10 schedule (Friday-Monday).
  • Weekend and on-call requirements vary; discuss during interview.
  • Use of PointCare/HCHB EMR system is required.
  • Must have recent home health experience; first-time home health therapists are not accepted.

Required Qualifications

  • Valid Physical Therapist license in Oregon.
  • Minimum of 1 year of Physical Therapist experience within the last year.
  • Experience working with adult patients in a home health setting.
  • Experience with Homecare Homebase (HCHB) EMR system.
  • BLS certification.

Additional Requirements

  • Must complete standard background checks, drug screening, and health documentation including:
    • Physical exam within 12 months.
    • Negative TB test or chest X-ray within 12 months.
    • Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
    • COVID-19 vaccination or declination.
    • Seasonal flu vaccination or declination.
  • Must provide two professional references.
  • Verification of work history for the last 7 years or at least two most recent employers.
  • Must review and sign various compliance, confidentiality, and policy documents prior to start.
  • Personal or auto insurance policy required for home health placements (policy must be current and under candidate’s name or list candidate as a driver).

Notes

  • No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic.
  • Local candidates are accepted.
  • On-call requirements vary by location.
  • Weekend requirements vary; discuss during interview.

This position is ideal for experienced physical therapists with a background in adult home health therapy who are comfortable with travel and working independently in patients' homes.
