Job Opportunity: Physical Therapist - Home Health

Job Description

• Provide skilled physical therapy services in a home health setting.
• Work 8-hour day shifts (no 4x10 or 3x12 shifts available).
• Patient ratio is 1:1, with up to 6 patient visits per day.
• Productivity standard: 30 points per day.
• Must have own transportation; occasional travel radius up to 50+ miles.
• Facility requires primary series COVID vaccination plus one booster.
• Charting system: EPIC/Oasis.
• Scrub color: Any.
• Parking: Free.
• Mileage reimbursed at IRS standard rates.
• Locals within 50-75 miles considered local and accepted.
• Pending license accepted.
• Facility may call off up to one shift every two weeks.
• No weekend shifts required.
• No on-call or floating between units.
• No block schedules accommodated.
• First-time travel therapists not accepted.

Required Qualifications

• Valid Physical Therapist license in Oregon.
• Minimum of 1 year of Physical Therapist experience within the last year.
• Experience in a Home Health setting.
• Experience with OASIS documentation.
• Completion of at least one travel assignment.

Compliance and Documentation Requirements

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO.
• Immunizations and health screenings required:

  • COVID-19 vaccination or declination.
  • Flu vaccine during flu season or declination.
  • Tetanus/Diphtheria/Tdap within 10 years or declination.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer.
  • Varicella vaccination or positive titer.
  • Hepatitis B vaccination or signed declination.
• 10-panel drug screen within 90 days of start (no instant tests).
• Respirator clearance required for travel candidates, updated annually.
• Background check within 45 days of start covering all locations lived/worked in past 7 years.
• Education verification and employment references required.
• Must complete required WorkRamp compliance and specialty modules prior to start.
• Badge photo required for identification.

Additional Notes

• This position is strictly for experienced home health physical therapists; no first-time travelers.
• No weekend or on-call shifts.
• Facility does not accommodate alternative shift schedules such as 4x10 or 3x12.
• Must be able to work independently in patients’ homes.
• Facility groups clients to minimize driving time but expect occasional long drives.
• Locals within 50-75 miles are accepted and treated the same as travelers.
