Registered Nurse - Hospice

Location: Basking Ridge, New Jersey
Employment Type: Travel Clinical Allied

Job Description

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop and implement individualized plans of care for patients and families.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods and document patient education.
• Manage case load by prioritizing care needs and revising care plans as necessary.
• Demonstrate knowledge of hospice regulations, pharmacology, and safe medication management, including medication reconciliation.
• Participate in interdisciplinary team (IDT) meetings every two weeks and communicate patient progress.
• Maintain a safe environment by following infection control measures and reporting infections.
• Participate in performance improvement programs, utilization review meetings, and staff meetings.
• Provide orientation, mentorship, and cross-training to new employees.
• Assist with agency growth through additional assignments and training.

Contract Length

• Approximately 26 weeks

Required Qualifications

• Valid Registered Nurse (RN) license in New Jersey
• Minimum 2 years of hospice nursing experience, including experience within the last year
• Experience in a home health setting
• BLS certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing

Preferred Qualifications

• 3 or more years of RN experience in home hospice

Education

• Associate's Degree in Nursing (ASN) or Diploma with RN License in New Jersey
