Registered Nurse (RN) – Medsurg/ER Swing

Location: Baudette, Minnesota
Contract Length: 13 weeks
Shift: 12-hour shifts, rotating day and night (6:45am–7:15pm and 6:45pm–7:15am)
Weekend Requirement: Every 3rd weekend (Friday night through Sunday night)
Holiday Requirement: Every 3rd holiday, integrated into permanent staff rotation

Unit Details

• Unit: Medsurg with ER Swing
• Beds: 9 Med Surg rooms, 3 ER patient bays
• Nurse-to-Patient Ratio: 1:5 with CNA support
• CNA Role: Hourly rounding, blood sugars, vital signs, patient care as scheduled
• Patient Types: Rehab swing bed, Med Surg patients with pain control, pneumonia, hypertensive crisis, bowel obstruction, some cardiac cases
• Respiratory Equipment: BiPAP, high flow oxygen, Hamilton T1 ventilator (patients requiring ventilation usually transferred to higher level of care)

Job Responsibilities

• Float between Medsurg and Emergency Department
• Admit, discharge, and transfer patients
• Titrate medications and drips (including insulin, amiodarone, cardizem, levophed)
• Start IVs and manage PICC lines/ports
• Participate in codes: cardiorespiratory, stroke, sepsis
• Respond to trauma situations
• Independently read and document EKG strips (Mindray monitors) and complete EKGs
• Work with minimal staffing; communicate directly with hospitalists
• Follow EMTALA regulations and coordinate patient transfers including flight transport

Required Certifications

• BLS (Basic Life Support)
• ACLS (Advanced Cardiovascular Life Support)
• PALS (Pediatric Advanced Life Support)
• TNCC (Trauma Nursing Core Course)

Required Experience & Skills

• Emergency Department experience required to float between Medsurg and ER
• Ability to titrate drips and manage complex patient care independently
• Comfortable working with minimal staff and adapting quickly
• Experience with Meditech EMR preferred

Scheduling & Orientation

• Schedule posted 4-6 weeks in advance by manager
• Every 3rd weekend and holiday rotation
• On-call is voluntary, occasionally required during high patient loads or low census
• Orientation typically one week, adjusted based on skill level

Additional Notes

• Nursing is the only 24-hour staffed department; ancillary departments are on-call nights and weekends
• Lead nurse is the most senior RN on schedule; minimal staffing includes 2 licensed nurses and 1 CNA per shift
• Scrubs provided, no specialty colors required
