Surgical Tech Position - Elmira, New York
Job Details
- Specialty: Certified Surgical Technician
- Location: Elmira, New York
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Shift: 7:00 AM – 3:30 PM
- Shift Options: 8, 10, or 12 hour shifts available
- Call Requirements:
- One weekday call required every 2 weeks
- Weekend call rotation required
Required Qualifications
- Surgical Technician with at least 1 year of experience, including experience within the last year
- BLS certification issued by the American Heart Association
Health and Safety Requirements
- Vaccinations and Immunizations:
- MMR: Positive IgG titer or 2 vaccine series required; must show results for each component
- Varicella: Positive IgG titer or 2 vaccine series required; documentation must include dates and results
- Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form required
- Influenza Vaccine: Required annually or specific declination form; mask required if declined
- Tdap: Preferred within last 10 years; if not available, specific declination form required
- TB Testing:
- Must have one of the following within the last 12 months:
- Two skin PPD tests (one within 12 months, one within 3 months) or
- One Quantiferon/Spot test within 12 months
- For past positive TB tests, chest X-ray within 12 months may be required
- Fit Test:
- Must be completed within the last 12 months prior to start
- Approved mask models specified; on-site fit testing available if needed
- Physical/Health Statement:
- Must be from within the last 12 months
- Must include physician credentials, clinic location, phone number, height, weight, vitals, Snellen eye chart, and a statement of clearance with no restrictions
Documentation and Onboarding Requirements
- Background Check:
- Must be completed within 30 days of start
- Includes SSN address trace, county and national criminal searches, FACIS 3 check
- Drug Screen:
- 9+ panel drug screen required within 30 days of start
- Identification:
- Valid, unexpired US driver’s license or state ID required
- Resume:
- Complete employment history for last 7 years (or since graduation) with education details
- References:
- Two professional references required:
- One supervisor reference from last 12 months
- One supervisor or peer reference from last 3 years
- Employment and Education Verification:
- Verification of most recent employer and initial education in the field required
- Additional Required Forms:
- Signed job description
- Personal information form
- Code of conduct acknowledgment
- Confirmation of assignment form
- Self-study packet documentation and quiz
- Sexual harassment acknowledgment
- Parking registration form
- TB questionnaire form
- Agency-specific background checks (7 manual checks required)
- Drug and tobacco acknowledgment
Optional/Additional Information
- COVID-19 vaccine proof or agency declination is optional
- Extended travelers beyond one year require reassessment form
