Description
Job Opportunity: Home Health/Hospice Registered Nurse (RN)
Location: Gunnison, Colorado
Employment Type: Contract
Contract Length: 13 weeks
Job Description
- Provide patient care and case management for home health, hospice, and palliative care patients.
- Conduct initial evaluation visits and regularly re-evaluate patients' nursing needs.
- Initiate and revise plans of care as necessary.
- Deliver specialized nursing services and preventive/rehabilitative nursing procedures.
- Prepare clinical and progress notes.
- Coordinate services and communicate changes in patient condition to physicians and other personnel.
- Counsel patients and families regarding nursing and related needs.
- Participate in in-service training programs.
Work Schedule
- Day shift.
- On-call requirements: 1-2 nights per week and 1-2 weekends per month.
- Approximately 36 hours per week.
Required Qualifications
- Valid Registered Nurse (RN) license in Colorado.
- Basic Life Support (BLS) certification.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 1 year of hospice experience within the last year.
- Home health nursing experience.
Additional Information
- This position involves working in a rural setting.
- Prior travel experience is required.
