Description
Physical Therapist (SNF) - Travel/Contract Opportunity
Location: Live Oak, California
Assignment Length: 13 to 26 weeks
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Start Date: ASAP
Job Description
- Provide physical therapy services in a Skilled Nursing Facility (SNF) setting.
- May include occasional weekend coverage and floating as needed.
Required Qualifications
- Valid Physical Therapist license in California.
- Minimum 1 year of Physical Therapist experience, including experience within the last year.
Compliance and Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID).
- Emergency contact information prior to start.
- Background checks including 7-year residential/work/school history, sex offender registry, and annual federal exclusions (EPLS/GSA/SAM, HHS/OIG).
- Physical exam within 1 year of start date.
- Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, a negative chest X-ray within 1 year is required.
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine series, or history of chickenpox)
- 9-panel drug screen within 1 year of start.
- Seasonal flu vaccination or declination during flu season (October 1 - March 31).
- Annual skills checklist and medication/pharmacology competency exam as applicable.
References
- 2 professional references required for travel, allied, or per diem acute care roles.
- 1 professional reference required for per diem non-acute care roles.
- References should be within 3 years of start date.
Additional Notes
- This is a travel/contract position requiring flexibility for occasional weekend shifts and floating duties.
- Prior travel experience is not explicitly required.
