PT Home Health - Portland, OR (Travel/Contract)


Job Details

  • Specialty: Therapy/Rehabilitation (Physical Therapist)
  • Location: Portland, Oregon
  • Employment Type: Travel/Contract
  • Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
  • Contract Length: 13 weeks
  • Work Setting: Home Health (visiting patients in their homes)
  • Patient Load: 1:1 ratio; up to 6 patients per day; 30 productivity points standard
  • Weekend Requirement: No
  • On-Call: No
  • Floating: No floating between units
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes, within 50-75 mile radius considered local
  • Guaranteed Hours: Facility may call off up to one shift every two weeks
  • Transportation: Traveler must have their own transportation; daily travel radius up to ~60 miles (60-90 minutes total drive time)
  • Attire: Business casual; scrubs allowed but not required; no jeans
  • Charting System: EPIC Remote Client (laptop), Rover app (phone), and OASIS documentation required
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Required Qualifications

  • Valid Physical Therapist license in Oregon
  • Minimum 1 year of Physical Therapist experience, including experience within the last year
  • Experience in a Home Health setting
  • Experience with OASIS documentation
  • Basic Life Support (BLS) certification

Compliance & Background Requirements

  • Background Check: Within 90 days of start, covering all locations lived/worked in past 7 years; must be updated annually
  • Drug Screen: 9-panel drug screen within 12 months of start; instant tests acceptable with attestation
  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO; must indicate ability to work with or without accommodation
  • Immunizations & Health Screenings:
    • TDAP vaccine within 10 years (declination accepted)
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
    • Seasonal Flu vaccine during flu season (declination accepted)
    • COVID-19 vaccination record preferred; declinations accepted with adherence to masking policies
  • Respirator Clearance: Required for travel candidates; annual medical clearance for respirator use
  • Fit Mask Testing: Required for travel candidates; completed onsite if no valid record within 1 year
  • State-Specific Checks: Oregon Abuse/Neglect Search if applicable; Washington WATCH and OBRA Registry searches for border state placements or as required

Training & Competency Requirements

  • WorkRamp Modules: Annual regulatory and specialty-specific modules required for travel caregivers; not required for home health at this facility
  • Competency Exams: Medication and specialty competency exams as applicable
  • Epic Training: Completion of EPIC modules required within orientation week (for applicable hospital systems)
  • Skills Checklist: Annual update required for applicable units

Additional Notes

  • Facility cannot accommodate 4x10 or 3x12 shifts
  • Traveler may be called off up to one shift per pay period
  • No floating between units expected
  • Must comply with float policy acknowledgment if applicable
  • Must provide passport-style badge photo with neutral background
  • Must complete system TB questionnaire prior to start and annually
  • Physical exam must be updated annually for placements in certain hospital systems (not required annually for home health unless specified)

This position is suitable for first-time travel therapists and locals within the specified radius.
